Does lovastatin (a Cholesterol-lowering drug) reduce the risk of heart attack?
Does lovastatin (a cholesterol-lowering drug) reduce the risk of heart attack? In a Texas study,
researchers gave lovastatin to 2,325 people and an inactive substitute to 2,081 people (average age
58). After 5 years, 57 of the lovastatin group had suffered a heart attack, compared with 97 for the
inactive pill. (a) State the appropriate hypotheses. (b) Obtain a test statistic and p-value. Interpret
I am with squatting dog. I get bored with all of this p-value junk, but statins have been shown to be good for prevention of a secondary event (such as a person who has already had one heart attack), but the evidence is still up in the air as to whether it really helps prevent a first heart attack. If it were me, though, and my cholesterol was bad, I would want to take it just because it’s a possible benefit.

The present study utilized a translational research approach to comprehensively evaluate the effects of pistachios on CVD. We employed a randomized crossover controlled-feeding study to evaluate the effects of two doses of pistachios, added to a lower-fat diet, on lipids and lipoproteins, apolipoprotein-defined lipoprotein subclasses, and plasma fatty acids. To investigate mechanisms of action, we measured serum cholesteryl ester transfer protein (CETP), indices of plasma stearoyl-CoA desaturase activity (SCD), and gene expression in isolated peripheral blood mononuclear cells (PBMCs). Total cholesterol (TC), LDL-C, non-HDL-C, apoB, and apoB/apoA-I decreased after both pistachio diets; and triacylglycerol and plasma SCD activity decreased after the 3.0 ounce pistachio diet (P < 0.05). Pistachios elicited a dose-dependent lowering of TC/HDL-C, LDL-C/HDL-C, and non-HDL-C/HDL-C (P < 0.01). We evaluated the effects of pistachios on expression of genes related to inflammation and lipid metabolism (TNFalpha, IL-1beta, IL-6, ICAM, VCAM, CETP, and LCAT) in PBMCs. Furthermore, we investigated the relationship between diet-induced change in CETP expression and change in serum CETP and plasma lipids/lipoproteins. The pistachio-rich diets significantly decreased IL-1beta expression compared to baseline (P < 0.05). Cholesterol, Atherosclerosis And Coronary Disease In The Uk, 1950-2000. $9.5 Cholesterol began to be accepted after the Second World War as a significant cause of atherosclerosis and associated conditions, such as coronary heart disease (CHD). This Witness Seminar, chaired by Professor Michael Oliver, discussed the increasing understanding of its basic science. Early epidemiological studies demonstrated the relationship between excess saturated fat consumption and elevated levels of cholesterol, although cholesterol alone did not explain all population differences among gender, class, or heritage. Work on lipoprotein metabolism pointed to the establishment of hyphercholesterolaemia as a major risk factor for CHD, culminating in the development of cholesterol-lowering drugs, particularly the successful statins, available in the UK from the early 1980s, and confirmed by randomized controlled trials. The role of diet in heart disease has remained controversial in the UK, notwithstanding the food industry’s introduction of functional foods such as cholesterol-lowering margarine. Later, recommended limits on the composition of dietary fat were agreed, and although extreme diets could reduce cholesterol, patient conformity remains difficult. Appendices on the diet-heart hypothesis and the development of lovastatin compliment the transcript. Some Pharmaceutical Drugs $6.14 Evaluates the carcinogenic risk to humans posed by the therapeutic use of thirteen pharmaceutical drugs, including eight benzodiazepines and related compounds used for the treatment of anxiety and as sedatives and anticonvulsants, three triphenylethylene anti-oestrogenic compounds developed for the treatment of breast cancer, and two cholesterol-lowering agents used to treat patients at high risk for cardiovascular disease. Evaluations are based on a critical assessment of all data available for these compounds, including abundant information on pharmacokinetic and pharmacodynamic effects, precise studies of exposure-response relationships, and findings that shed light on mechanisms of carcinogenic action. The most extensive monograph evaluates the large body of data on tamoxifen, which has been used for almost two decades as the first-line endocrine therapy for postmenopausal women with advanced metastatic breast cancer. Tamoxifen is also used as adjuvant therapy in patients with breast cancer and is being tested for use as a preventive agent. The evaluation found sufficient evidence in humans for the carcinogenicity of tamoxifen in increasing the risk for endometrial cancer, and conclusive evidence that tamoxifen reduces the risk for contralateral breast cancer in women with a previous diagnosis of breast cancer. Evidence for the carcinogenicity of tamoxifen in other organs was judged inadequate. Of the benzodiazepines, diazepam, which is the most widely prescribed, received the most extensive evaluation. Evidence reviewed suggested lack of carcinogenicity to the breast and inadequate evidence for carcinogenicity at other sites in humans. We compare frequencies of large-molecule leaks in the susceptible, large, high-pressure aorta, the rarely susceptible pulmonary artery and the immune inferior vena cava. The average horseradish peroxidase (HRP) leakage frequencies from 35 male Sprague Dawley rats are, respectively, 7.38+/-1.77, 3.34+/-0.87, 0.441+/-0.158 per 104 cells. Neither tracer circulation time nor acute blood pressure affects these numbers.;Our group’s earlier theoretical work explained historic HRP localized spot growth vs circulation time data with a water and macromolecular transport theory. Our local theory based on the idea that the intima compresses under DeltaP causing the endothelium to block IEL fenestrae, thereby lowering wall hydraulic conductivity Lp, clarified anomalous Lp data. Combining these theories, Chapter 3 predicts that only with initmal compaction do higher acute pressures mean smaller spots with shorter rise times. Our subsequent experiments verify these predictions.;Although Huang et al.’s axisymmetric convection-diffusion transport theory predicts HRP concentration near localized leaks vs r, z and t, experimental spot size tests correspond to integrating these predictions through the tissue. A more stringent test would be to compare predictions with 3D HRP concentration profiles. Chapter 4 uses confocal microscopy and an HRP substrate with fluorescent reaction products to observe such profiles. Our theory can mimic these experiments, including the washing period before fixation and we successfully compare experiment with theory. The focal spread of macromolecular tracers in vessel walls: Frequency and effect of intima compaction and blood pressure. $108 Atherosclerosis appears to begin when a transmural pressure (DeltaP)-driven water flow advects low-density lipoprotein cholesterol from the lumen into the subendothelial intima around rare, isolated endothelial cells with temporarily widened junctions. We compare frequencies of large-molecule leaks in the susceptible, large, high-pressure aorta, the rarely susceptible pulmonary artery and the immune inferior vena cava. The average horseradish peroxidase (HRP) leakage frequencies from 35 male Sprague Dawley rats are, respectively, 7.38+/-1.77, 3.34+/-0.87, 0.441+/-0.158 per 104 cells. Neither tracer circulation time nor acute blood pressure affects these numbers.;Our group’s earlier theoretical work explained historic HRP localized spot growth vs circulation time data with a water and macromolecular transport theory.
